Clinical Records & Data Retention
Brief clinical records and session notes may be maintained as part of professional practice, ethical standards, and insurance requirements. Clinical documentation is retained for up to seven years following the conclusion of services. The Misneach Centre does not produce reports or documentation for legal proceedings, court cases, schools, or third-party agencies. At the end of the applicable retention period, documentation is securely destroyed and permanently deleted.
Confidentiality & Access to Records
All clinical systems and records are protected by appropriate security measures, including password-protected and restricted-access storage. Access to clinical records is strictly limited to your treating therapist and the Clinical Director where clinically appropriate. Administrative personnel have limited operational access only and do not have access to clinical notes.
